no dude ive been looking on google and yahoo for like 10 minutes. all i can find is the software you download. but i have an alternative, you can go on Microsoft excel and format the page to how many slots you want. make a slot for all of the things you mentioned down the side, and make 19 slots across, one for each hole and one for your name
